I have long preferred chunky to fat necks. Two of my favorite builds, a Tele and Strat, have full 1" baseball bat profiles. In recent months I've developed a sharp arthritic pain in the big joint of my left index finger. Its actually low on the knuckle, into the hand. I'm ok playing riffs and small 2-3 note shapes but boy does it say howdy on barre chords and bigger stretches. Its not so much an issue on some of my other necks that most would call chunky. Boatneck, Warmoth 59 Roundback. It'll twinge here and there, but not like it does with the Louisville sluggers.
I hope I don't have to reshape these necks. ☹

Have you tried playing with your thumb placement? That can change the posture of all the other joints.
I don't understand arthritis all that well, but it seems like a skinnier neck would mean the joint would have to bend more... or is it the straightening that is more painful?

This is so new that I haven't really scrutinized the anatomy of it. I most definitely feel it if I do the "devil horns" stretch with index finger and pinky pointing out.

I feel your pain, brother. About 10 years ago it got to the point that playing on a thin neck (front to back) caused my hand to cramp up--the fat thumb muscle in the palm. It was excruciating pain. Anything more than about 30 minutes of guitar playing and I was in the hurt zone.
Fatter necks solved that issue for me. But now I have flexor tendinitis in the little finger, and that is seriously preventing me from playing like I used to.
Lap steel, here I come!
And after that--stamp collecting!

one thing to try is to strengthen the surrounding muscles by using a hand exerciser. Just a suggestion as sometimes these pains come and go
