I think we've all seen watered down criticisms and definitions that attempt to lessen or deflect culpability. An operation in which the outcome for the patient is, "suboptimal," for instance.
Saw a new one today. New to me, anyway. When an elderly patient wanders off from an assisted care facility, only to be found hours later, frozen stiff or face down in a water filled ditch. The care industry terms this wandering "an elopement".
"Hello, Mrs. Henderson? Wonder Care here ... Your 90 year old dad eloped earlier today, setting off some really annoying door alarms...... yes, we know it's 15 degrees outside. The surveillance video shows him without a jacket, but carrying a fistful of pancakes."

Not hard to imagine the scenario playing out.
My mother, at the age of 90, and in the grips of dementia living in a care facility would be concerned about needing to get home before dark … because her mother would be worried about her.
This care center was essentially a “lock down” facility. It had to be due to this very reason.

After my mom died, my disabled dad lived with us for 2-1/2 years. After that, he became wheelchair bound and quite frail. We put him in the least objectionable rest home in town and I'd go visit him after work most days, bringing the wife and our two young children at least once a week to see "grampa". There was one dementia patient in that facility who was still quite physically fit. "Stan", his name was. You'd start a casual conversation with him and think to yourself, "why is he in here?" Right about then, he'd steer the conversation off in some direction that made it quite clear why he was in there.
One day, as I neared the facility parking lot, I saw "Stan", dressed in his pajamas, slippers and bathrobe (but no pancakes), striding purposefully up the road in the opposite direction. He'd evidently staked out one of the exit doors, waited for some visitor to enter or leave and slipped out.
When I informed a staff member of this "elopement", they freaked out and immediately set off in hot pursuit.

Lest we forget the granddaddy of them all: "Alternative facts". I just about snorted half a beer through my nose when I heard that one.
