Hi. I feel silly asking this question at such a late date. But better late than never.
I have always held my pick between the ball of my thumb and the ball of my index finger. However, I noticed that most good players seem to hold the pick between the ball of the thumb and the SIDE of the Index finger. I am trying to remember to hold it this way as it seems I can pick faster and more accurately this way. Hard to hold at first, but it feels more natural as time goes by.
How do you hold yours?

I think whatever works for you is fine. I can't quite visualize what you are describing but my method is close to the description in this video. Basically I look for the method this results in the most efficient movement. The best result is one where it looks like you hand is hardly moving.

Similar to the picture, but with a teardrop pick and more of the tip sticking out.
Edit: Actually, my index finger points more downward towards the string, with the pad/tip making more contact than in the picture. This might be why I prefer smaller picks...

I've never really thought about it, but I seem to alternate between what the picture shows...sometimes thumb and index finger, and sometimes thumb and middle finger.

I pick/strum with the fat round edge. I'm also a lefty that plays "orthodox".

Thumb and side of finger usually. I think EVH used thumb and tip of 2nd (social) finger.
