Does anyone have thoughts on how Tommy Iommi makes that opening diad (I think it's two notes) go from dissonance to something a little easier on the ear? The tune crossed my path this morning and I thought, 'oh, he's doing something clever."

Not exactly sure of what you're speaking of but it makes me wonder if it could be a byproduct of his 2 missing fingertips.

It's the duuuuuh-duuuuh, right after the two opening bars of the bass drum. The line goes from a two notes at a very dissonant interval to unison. Listening closely, it's starting to sound like a double-tracked guitar (with some happy mistakes in matching the notes.)

It also sounds to me like it's not double tracked, but doubled via a short single delay. (doubling delay)

Sounds either doubletracked, or...
He has the A string tuned to E and he's pulling it sharp behind the nut and releasing it while striking the E and A (tuned to E) strings. I just tried that and it sounds the same.
It doesn't sound like a delay to me because there's a constant E note throughout.

I believe it is double tracked. If you listen closely you can hear feedback towards the end of the sound just before it repeats. The feedback is only in the right speaker. If it were not double tracked it would be in both speakers. To me, it sounds like 2 guitars, recorded on 2 different tracks, each doing the same thing on a single string. They both start off in the right speaker and then one of them is panned to the left. Then it repeats starting on the right and panning to the left.
